That's Lucrezia, Agatha's mum, in a heavily modified clank body Tarvek used to house his Sister in. She acquired that back in 2006 in comic terms.

Thanks. I'd forgotten what that version of Lunevka looked like. Did she ever have four arms before?

It doesn't look like she has four arms now--in Panel #4, at least.

Staring at it more closely, I think you're right. The wrist guard somehow looked like a second left arm to me.

I parsed that as two arms on her left side, too. If so, they're close together, not with the vertical separation and second set of shoulders Steelgarter has.
